Job Information:

Alaris Health at Essex, a 212 bed skilled nursing Center in Irvington, New Jersey, seeks a Nurse functioning at a Masters or Post-Masters level meeting the medical, bio-psycho-social, and functional needs of an identified population that include identification of needs through appropriate assessment, diagnostic, and management activities. The Nurse Practitioner will report to the Regional Director of Clinical Services. The Nurse Practitioner is recognized as an advanced practice nurse in accordance with State Regulations and certification through a nationally recognized credentialing Center. The scope of practice is defined to a particular group of clients as defined under the Nurse Practitioners scope of practice and/or collaborative practice agreements with other healthcare providers.

Additional responsibilities include:

  • Assessing acutely ill residents to decrease unnecessary re-hospitalizations and emergency room visits
  • Optimization of length of stay for defined resident groups
  • Assessing residents for potentially at risk medical events such as wounds, strokes, falls et al. through unit rounds, attendance at meetings if appropriate and general referral by staff
  • Working with the Center to communicate the health status of residents to families, physicians and health team professionals
  • Providing education to staff, families and others on a routine basis based on identified educational needs from administrator, as well as other members of the health care team
  • Assisting in the discharge planning of specific residents with higher need acuity back to the community when desired and possible
  • Improving overall customer satisfaction through direct resident, family and physician contact
  • Assisting and directing the Center in standardization of clinical protocols, care regimens and formularies
  • Outcomes analysis and management for identified disease states.
  • Maintaining cohesive public and professional relationships, including community, resident and family and physician services
  • Developing short and long-range plans for the nursing department that are compatible with those of the facility
  • Interacting with residents, families, visitors and employees
  • Specializes in the Clinical Management, in conjunction with the physician and other health care professionals, to stabilize the residents care, treatment and services
  • Specific programs to be identified by the management team in the applicable Center and an addendum attached to the job description may outline more specific criteria for performance;
  • Carrying out other tasks as requested in situations where hands-on intervention/participation may be required
Job Requirements:
  • The Nurse Practitioner must be a graduate of an accredited school of nursing, college or university with a current Registered Nurse licensure by the New Jersey State Board of Nursing who has completed graduate-level education. Either a Master’s or a Doctoral degree is appropriate
  • Full command of the English language (written, read and spoken)
  • Must be able to demonstrate leadership skills, organization, provide a positive attitude, direction, and discipline for the clinical department
  • Must possess the ability to work independently, problem solve and make decisions as necessary
  • Must possess the ability to positively interact with personnel, residents, family members, visitors, government agencies/personnel and the general public
  • Must be able to develop and implement programs, policies and procedures, etc., which are necessary to assure that the highest degree of quality of care is consistently provided to the residents
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to long term care administration
  • Must possess through knowledge of licensing and certification regulations
  • Must provide annual verification of a negative TB skin test
